Notes edit edit source It is possible to teleport to the secret room or super secret room. It is not possible to teleport to the I AM ERROR room. It is not possible to teleport to the crawl space for the floor or the black market.
The Binding of Isaac: Afterbirth brings 95 new items to the playing field with the DLC that all have varying effects!
From powerful usable items to interesting passive items, the expansion introduces many items that will increase the amount of fun you have when playing the game!
Usable Items

Box of Friends | Duplicates all of the player's Familiars for the duration of the room. |
D12 | Re-rolls all rocks in the room into other objects, such as bomb rocks, poop, pots, etc. |
D7 | Re-rolls the current room and brings back all the enemies, allows for farming of certain rooms for rewards. |
D8 | Re-rolls all of the player's stats. |
Diplopia | Duplicates all pickups and items that are on pedestals in the room. |
Friendly Ball | Throws a ball that kills an enemy and captures it. You can then throw it and it will be charmed until it dies, which will last forever even if you move to other floors. |
Glass Cannon | Reduces the player's health to half a heart (removes soul/black hearts) when used and fires a giant piercing spectral tier that deals a significant amount of damage. It scales with the player's damage. |
Glowing Hour Glass | Rewinds time to put the player in the very last room they were in, undoing anything that happened in the room where the item was used. Items obtained or lost, pickups that were dropped, stat changes that occured, will all be undoed. Does not affect item charges so you cannot infinitely use this item simply by using it. |
Jar of Flies | Stores a blue fly each time an enemy is killed, up to a maximum of 20. Can be used to release all the flies. |
Kidney Bean | The player farts and charms nearby enemies. |
Mega Bean | Freezes all enemies in the room for a few secnds, deals some damage, and poisons any enemies near the player. Then, it sends a wave of spikes out that deam damage across the room in the direction the player is facing. |
Mega Blast | The player fires a giant laser akin to the one used by Mega Satan, dealing massive damage, lingering through rooms and floors, and pushes the player back as long as the laser is firing for the 15 second duration. |
Mine Crafter | Places an explosive TNT barrel that can be pushed. Can be used again to explode the TNT barrel. |
Mom's Box | Drops a random trinket on the ground, increases Luck by 1, and doubles the effect of trinkets. |
Placebo | Copies the effect of the current pill you are holding, similar to Blank Card. |
Tear Detonator | Detonates any tears on screen and causes each one to split into 6 tears in a circle, like Tammy's Head. |
Teleport 2.0 | Teleports the player to a random room that has not been explored yet. If there are none, then it will teleport to a secret room. If there are none of those, then it will teleport the player to a Devil Room or Angel Room. If you have already been there, then it will teleport you to the I AM ERROR room. |
Ventricle Razor | Places an orange trapdoor and a blue trapdoor, both of which can be used to end up where the other trapdoor is located. Acts like the portals from the game Portal. |
Wooden Nickle | Has a 50% chance to drop a random coin. |

8 Inch Nails | +1.5 Damage. Replaces tears with nails, which increases knockback. |
Athame | Whenever the player takes damage, a black ring briefly appears around the player and damages any enemies it comes in contact with, with a chance of dropping a black heart whenever it kills an enemy this way. |
Betrayal | Whenever the player takes damage, a charm effect is applied to all enemies in the room. |
Binky | +1 Tears, +1 Soul Heart, and -1 size. |
Black Powder | The player leaves behind a trail of black powder, that when drawn in a circle will spawn a pentagram that deals damage to enemies that stand on it. |
Bomber Boy | +5 Bombs, all bombs now explode in a plus-shaped pattern. |
Bumbo | Floating familiar that picks up nearby coins. Every 6 coins (bar the last form, which needs 12) he will evolve into a new form. Level 2 removes his floating, Level 3 will make him fire tears in the same direction as the player, and Level 4 chases enemies, sometimes dropping bombs and picking up any coins to drop random pickups. |
Bursting Sack | Spider enemies no longer target or deal damage to the play.er |
Cambion Conception | After taking damage a certain number of times, the player will spawn a permanent demonic familiar. This can happen 4-6 times at most. |
Car Battery | All usable items activate twice. |
Censer | Gives the player a censer familiar which creates an aura that slows any enemies inside. |
Chaos | All item pools are randomized, and when picked up it will drop 1-6 random pickups on the floor. |
Charged Baby | Will drop a battery every 4 rooms and has a chance to freeze all enemies in the room when hit. |
Circle of Protection | Gives the player a halo around him, dealing damage to any enemies that come in contact with the ring. Any projectiles that enter have a chance to return a homing tear back at the enemy. |
Continuum | +2.25 Range, +1.5 tear height. The player's tears travel through walls and appear on the opposite wall. |
Crack Jacks | +1 Red Heart Container, drops a random trinket. |
Crown of Light | Gives the player a crown that glows blue until the player takes damage, in which it will deactivate and turn gray. While the player has no empty heart containers, your tears will be blue diamond tears that deal double damage. |
Curse of the Tower | When the player is hit, 6 troll bombs will spawn in the room. |
Dead Eye | The player's damage increases each time an enemy is hit, resetting on a miss. |
Deep Pockets | Allows the player to carry two pills or cards. |
Eden's Blessing | +0.7 Tears, gives a random item at the start of the next run. |
Empty Vessel | When the player has no red hearts, grants flight and the random chance to activate an invincibility shield for a few seconds. |
Epiphora | Shooting in one direction increases the player's fire rate. If you stop firing or you change the direction of your tears you will reset your fire rate. |
Explosivo | The player's tears have a chance to stick to enemies and explode. |
Farting Baby | Grants the player a familiar that follows the player and blocks shots. Will occasionally fart that ither knocks back, charms, or poisons enemies when it successfully do so. |
Fate's Reward | Grants the player a familiar that follows them and shoots tears that copies the player's tear effects and damage. |
Friend Zone | Grants the player a white orbiting fly that deals contact damage to enemies. |
Fruit Cake | The player's tears gain a random effect on every single shot. |
GB Bug | Grants the player a familiar that bounces around the room, applying random status effects to any enemies it passes over and rerolls any pickups that it flies over. |
Glitter Bombs | +5 Bombs, makes the player's bombs have a chance to drop random pickups or charm enemies when they explode. |
God's Flesh | The player's tears now have a chance to shrink enemies, that will run away from the player and take damage when walked over. |
Head of the Keeper | The player's tears become coins and have a chance to drop pennies on a successful hit. |
Holy Light! | The player's tears have a chance to become holy tears, which glows white and will create a beam of light on the enemy dealing damage to any enemy that touches it. |
Host Hat | Gives the player a host familiar that sits on their head, granting immunity to explosions and having a random chance to block enemy projectiles and retaliate with three tears. |
Immaculate Conception | Upon picking up a certain amount of health, spawns a permanent angelic familiar. |
Incubus | Grants the player a familiar that follows them and shoots tears that copies the player's tear effects and damage. |
Key Bum | Grants a player a Key Master familiar that follows the player and takes keys, giving random chests in return. |
Kidney Stone | Randomly while firing tear sthe player will stop firing, turn red, and then shoot out a large burst of tears along with a kidney stone that deals a large amount of damage. |
Lil' Chest | Spawns a chest familiar that drops random pickups every few rooms. |
Lil Gurdy | Grants the player a familiar that will charge in a direction when the player holds down the fire button for a while, dealing damage to enemies it comes in contact with. |
Lil' Loki | Grants the player a familiar that follows the player and shoots out 4 tears in a cross pattern. |
Lost Fly | Grants the player a fly familiar that attaches to the first wall or obstacle it comes in contact with, moving along the objects/walls and dealing damage to enemies it touches. |
Lusty Blood | Each time the player kills an enemy, their damage increases for the current room. |
Marked | Tears will now fire at the red mark on the ground automatically without needing to press the fire button. |
Maw of the Void | +1 Damage. After firing for 3 seconds straight, after releasing the player will create a black ring around them similar to the Athame item that will deal damage to any enemies inside of it. Any enemies killed by the item have a chance of spawning a black heart. |
Milk! | Grants the player a familiar glass of milk that will follow them until they take damage, breaking and giving the player a tears upgrade for the rest of the room. |

More Options | Two items will now spawn in Treasure Rooms, although only one can be taken. |
Mr. Dolly | +0.7 Tears, +5.25 Range, +0.5 Tear Height, spawns three random types of hearts on the floor when picked up. |
Multidimensional Baby | Grants the player a familiar that will follow the player's movements at a 2.5 second delay. Shooting through the baby will double your tears and increase their damage. |
My Shadow | Each time the player is damaged, fear is inflicted on all the enemies in the room and a black charger spawns that will attack enemies and die if it takes enough damage. |
Night Light | The player will now have a cone of light in front of them that slows enemies and projectiles, lighting up any darkness as well. |
No. 2 | Continuously firing tears causes the player to drop a butt bomb, that deals damage to all enemies in the room and dazes them. |
Papa Fly | Grants the player a blue fly familiar that follows the players movements on a 3 second delay, firing tears at any nearby enemies. |
Pay to Play | All doors that requires a key now requires a coin to open instead. |
PJs | +4 Soul Hearts, fully restores all your health. |
Pupula Duplex | The player's tears turn into a wide arc and makes them spectral tears. |
Purity | Boosts the stats of the player depending on the aura around him, will disappear on taking damage and reappears as a random color in the next room. Red = Damage, Blue = Tear Rate, Yellow = Speed, Orange = Range. |
Obsessed Fan | Grants the player a purple fly familiar that will follow the player's movements on a three second delay, dealing contact damage to enemies. |
Restock | All shops will now instantly restock items whenever you buy them. |
Rune Bag | Grants the player a bag familiar that follows the player and spawns a random rune every three rooms. |
Sack Head | Greatly increases the chance to find sacks from random pick-ups or replacing coins. |
Scatter Bombs | +5 Bombs, the player's bombs explode into 2-4 tinier bombs. |
Seraphim | Grants the player an angelic familiar that will fire tears that are like the Sacred Heart tear buff. |
Serpent's Kiss | Tears now have a random chance to apply poison to an enemy and the player will also apply poison to enemies on contact. Enemies killed by this poison have a high chance of dropping a black heart. |
Sticky Bombs | +5 Bombs, bombs now stick to enemies and spawn 2-4 blue spiders if they kill them. |
Spear of Destiny | The player now holds a spear in front of them which deals double his tear damage in contact damage and sometimes inflicts fear. |
Spider Mod | Displays health of enemies and the player's tear damage in numbers and will inflict a random status effect on any enemy it comes into contact with. Also can spawn a blue spider upon clearing a room. |
Succubus | Grants the player a demonic familiar that will bounce around the room with a giant black aura around it, dealing damage to any enemies within the aura. While the player is inside the aura, they gain a 1.5x damage multiplier. |
Sworn Protector | Grants the player an angelic familiar that will orbit the player and block enemy shots, with a chance of dropping an eternal heart whenever it does so. |
Tech X | Tears are now replaced with a chargeable circular laser ring that pierces through enemies and deals damage to enemies within the ring. |
The Wiz | Tears fire out of both eyes diagonally and will cause all tears to travel above obstacles. |
Toxic Shock | When entering a room all enemies will be poisoned and the player's damage is doubled. Killing enemies leaves a small pool of creep on the ground that will damage enemies. |
Tractor Beam | The player now has a blue beam that travels to the end of the room in the direction the player is facing. Tears will now travel along this path, even when you change the direction that you are facing. |
Zodiac | Gives the player a random zodiac item effect every floor. |
